I partner with you to identify the few high-impact actions most likely to benefit you—grounded in evidence, tailored to what matters most to you, and realistic within your day-to-day life.

Your partner in chronic condition self-management

Together, we’ll build self-management skills and a practical, flexible approach that fits your life and adapts as your health and circumstances change.

So you have more room for what matters most.

The day-to-day actions that support living as well and as fully as possible with a chronic condition.

Most of these actions happen outside of healthcare visits in everyday life and can include:

  • following treatment plans and lifestyle recommendations

  • monitoring symptoms or health numbers

  • navigating the condition’s impact on daily life

WHAT IS CHRONIC CONDITION SELF-MANAGEMENT?

Effective chronic condition self-management can meaningfully improve health and quality of life, but there can be more to do than is realistic to take on all at once, which can feel overwhelming.

HOW I HELP

A focused, realistic approach that adapts to real life

Making sense of it

“I can’t always tell what information to trust or how the pieces fit together.”

We sort through the recommendations you’ve received, information you’ve read, and symptoms or numbers you’ve tracked, exploring what they mean in the context of your health and everyday life.

Deciding where to focus

“There’s more to manage than I realistically can.”

We identify the few high-impact actions most likely to benefit you—grounded in evidence and tailored to what matters most to you.

Tailoring to real life

“Between work, family and everything else, my days are already full.”

We translate those actions into realistic steps that take your energy, schedule and responsibilities into account,so you have a little more room for the rest of your life.

Adjusting along the way

“I feel like I’m starting over every time something changes.”

Together, we’ll build self-management skills and a practical, flexible approach that adapts as your health and circumstances change.

Adults living with a wide range of chronic conditions

WHO I HELP

Including heart and metabolic conditions, neurologic conditions, respiratory conditions, bone and joint conditions, chronic pain, cancer, autoimmune conditions, mental health conditions, and more.

Whether the impact is felt day to day, reflected in your numbers, or both – and whether you are managing one condition or several.

WHY WORK WITH ME

Lived experience

I’ve lived with chronic conditions most of my life.

I know what it’s like to:

  • feel like health needs hijack your life

  • have very real symptoms dismissed, especially by people you expect to support you

  • get multiple recommendations that are each “just 10 minutes” but together add up to so much that you end up doing nothing

Expertise brought together in one place

I bring that lived understanding together with expertise in:

  • chronic condition self-management

  • evidence translation

  • behavioral science

  • lifestyle medicine

  • clinical exercise physiology

These areas of expertise are usually spread across different professionals.

What do you wish you had a little more room in your life for?

ABOUT ME

For more than 15 years, my work has focused on preventing and managing chronic health conditions. A major part of that work has been designing, implementing and evaluating self-management programs that help people build the skills and strategies they need to manage their health in everyday life. These programs have been delivered in person, by phone and text, and through digital health apps.

I've worked with people across the lifespan living with a wide range of physical, cognitive and mental health conditions. They've come from many cultural and socioeconomic backgrounds, with different responsibilities, resources and access to care. Those differences matter: even when the evidence or recommendation is the same, applying it in everyday life can look very different from one person to another.

My experience spans academic, community, startup, pharmaceutical and health-plan settings. Alongside my work with individuals managing their health, I've developed programs and solutions for healthcare providers, caregivers and family members.

I hold an MS and PhD in Exercise Physiology, focused on preventing and managing chronic conditions, and a BS in Psychology. I'm a Board-Certified Lifestyle Medicine Professional (DipACLM) and have authored or co-authored 20 peer-reviewed publications.
